Risk-Based Thinking: Managing the Uncertainty of Human Error in Operations
Society at large tends to misunderstand what safety is all about. It is not just the absence of harm. When nothing bad happens over a period of time, how do you know you are safe? In reality, safety is what you and your people do moment by moment, day by day to protect assets from harm and to control the hazards inherent in your operations. This is the purpose of Risk-Based Thinking, the key element of the six building blocks of Human and Organizational Performance (HOP). Generally, HOP provides a risk-based approach to managing human performance in operations. But, specifically, Risk-Based Thinking enables foresight and flexibility-even when surprised-to do what is necessary to protect assets from harm but also achieve mission success despite ongoing stresses or shocks to the operation. Although you cannot prepare for every adverse scenario, you can be ready for almost anything. When Risk-Based Thinking is integrated into the DNA of an organization's way of doing business, people will be ready for most unexpected situations. Eventually, safety becomes a core value, not a priority to be negotiated with others depending on circumstances. This book provides a coherent perspective on what executives and line managers within operational environments need to focus on to efficiently and effectively control, learn, and adapt.

Handbook of Safety Principles
Presents recent breakthroughs in the theory, methods, and applications of safety and risk analysis for safety engineers, risk analysts, and policy makers Safety principles are paramount to addressing structured handling of safety concerns in all technological systems. This handbook captures and discusses the multitude of safety principles in a practical and applicable manner and is organized by five overarching categories of safety principles: Safety Reserves; Information and Control; Demonstrability; Optimization; and Organizational Principles and Practices. With a focus on the structured treatment of a large number of safety principles relevant to all related fields, each chapter defines the principle in question and discusses its application as well as how it relates to other principles and terms. This treatment includes the history, the underlying theory, and the limitations and criticism of the principle. Several chapters also problematize and critically discuss the very concept of a safety principle. In addition, the book treats issues such as: What are safety principles and what roles do they have? What kinds of safety principles are there? When, if ever, should rules and principles be disobeyed? How do safety principles relate to the law; what is the status of principles in different domains? The book also features: * Insight from leading international experts on safety and reliability * Real-world applications and case studies including systems usability, verification and validation, human reliability, and safety barriers * Different taxonomies for how safety principles are categorized * Breakthroughs in safety and risk science that can significantly change, improve, and inform important practical decisions * A structured treatment of safety principles relevant to numerous disciplines and application areas in industry and other sectors of society * Comprehensive and practical coverage of the multitude of safety principles including maintenance optimization, substitution, safety automation, risk communication, precautionary approaches, non-quantitative safety analysis, safety culture, and safety management The Handbook of Safety Principles is an ideal reference and resource for professionals engaged in risk and safety analysis and research. This book is also appropriate as a graduate and PhD-level textbook for courses in risk and safety analysis, reliability, safety engineering, and risk management offered within mathematics, operations research, and engineering departments.

Case Studies in Public Health
Case Studies in Public Health contains selected case studies of some of the most important and influential moments in medicine and epidemiology. The cases chosen for this collection represent a wide array of public health issues that go into the makeup of what can be termed the New Public Health (NPH), which includes traditional public health, such as sanitation, hygiene and infectious disease control, but widens its perspective to include the organization, financing and quality of health care services in a much broader sense. Each case study is presented in a systematic fashion to facilitate learning, with the case, background, current relevance, economic issues, ethical issues, conclusions, recommendation and references discussed for each case. The book is a valuable resource for advanced students and researchers with specialized knowledge who need further information on the general background and history of public health and important scientific discoveries within the field. It is an ideal resource for students in public health, epidemiology, medicine, anthropology, and sociology, and for those interested in how to apply lessons from the past to present and future research.

Fundamentals of Wearable Computers and Augmented Reality
Data will not help you if you can't see it where you need it. Or can't collect it where you need it. Upon these principles, wearable technology was born. And although smart watches and fitness trackers have become almost ubiquitous, with in-body sensors on the horizon, the future applications of wearable computers hold so much more. A trusted reference for almost 15 years, Fundamentals of Wearable Computers and Augmented Reality goes beyond smart clothing to explore user interface design issues specific to wearable tech and areas in which it can be applied. Upon its initial publication, the first edition almost instantly became a trusted reference, setting the stage for the coming decade, in which the explosion in research and applications of wearable computers and augmented reality occurred. Written by expert researchers and teachers, each chapter in the second edition has been revised and updated to reflect advances in the field and provide fundamental knowledge on each topic, solidifying the book's reputation as a valuable technical resource as well as a textbook for augmented reality and ubiquitous computing courses. New Chapters in the Second Edition Explore: * Haptics * Visual displays * Use of augmented reality for surgery and manufacturing * Technical issues of image registration and tracking * Augmenting the environment with wearable audio interfaces * Use of augmented reality in preserving cultural heritage * Human-computer interaction and augmented reality technology * Spatialized sound and augmented reality * Augmented reality and robotics * Computational clothing From a technology perspective, much of what is happening now with wearables and augmented reality would not have been possible even five years ago. In the fourteen years since the first edition burst on the scene, the capabilities and applications of both technologies are orders of magnitude faster, smaller, and cheaper. Yet the book's overarching mission remains the same: to supply the fundamental information and basic knowledge about the design and use of wearable computers and augmented reality with the goal of enhancing people's lives.

The Fearless World of Professional Safety in the 21st Century
Professional safety is in danger of extinction. Safety professionals have become complacent and unfocused, ignorantly relying on an 80-year-old paradigm. Lazy gimmicks are substituted for the hierarchy of controls meant to be the foundation of the profession. A $10,000 investment in posters makes zero improvement in safety; a $10,000 investment in machine guarding upgrades can save lives. By blending philosophy, history, and psychology, The Fearless World of Professional Safety in the 21st Century is revolutionary, offering an innovative approach with creative solutions to move a safety program past the malarkey that has devalued professional safety for decades. Using humor and professional experience within a discussion of historical events and published scientific findings, Scott Gesinger explores the history of how current safety practices developed and why these must change if the profession is to survive the 21st century. He discusses new professional philosophies based on best practices in industry, historical examples, scientific research outside of safety, and proven approaches from other disciplines which can successfully guide safety professionals into the future. Gesinger provides a book for every safety professional, that is candid, plain-speaking and eminently approachable, while at the same time provides information that is new, challenging, and engaging.

Training and Assessing Non-Technical Skills: A Practical Guide
Providing a practical guide to the training and assessment of non-technical skills within high-risk industries, this book will be of direct interest to safety and training professionals working within aviation, healthcare, rail, maritime, and other high-risk industries. Currently, each of these industries are working to integrate non-technical skills into their training and certification processes, particularly in light of increasing international regulation in this area. However, there is no definitive guidance to assist practitioners within these areas with the design of effective non-technical skills training and assessment programs. This book sets out to fully meet this need. It has been designed as a practically focussed companion to the 2008 book Safety at the Sharp End by Flin, O'Connor and Crichton. While Safety at the Sharp End provides the definitive exploration of the need for non-technical skills training, and examines in detail the main components of non-technical skills as they relate to safe operations, the text does not focus on the nuts and bolts of designing training and assessment programs. To this end, Training and Assessing Non-Technical Skills: A Practical Guide provides an extension of this work and a fitting companion text.

Health Systems Improvement Across the Globe: Success Stories from 60 Countries
Following on from 2015's Healthcare Reform, Quality and Safety: Perspectives, Participants, Partnerships and Prospects in 30 Countries, this book encompasses a global perspective on health care while shifting the focus from reform to showcasing success stories of healthcare systems worldwide. It provides explanations of why various facets of healthcare systems work well in different contexts and offers the reader alternative models for consideration. The book features contributions from 70 countries, going much further than the common practice of focusing on affluent Western nations, to provide a comprehensive exploration of the success of healthcare systems globally. The majority of literature on health-sector improvement attempts to address the problems within systems, relating the errors that can and do occur, for example, and offering solutions and preventative strategies. This book of country case studies will approach the enhancement of health systems, patient safety and the quality of care in a new and innovative way, comprehensively surveying and synthesizing the success stories of healthcare systems around the world, utilizing Hollnagel's Safety-II approach to acknowledge the importance of exploring what goes right, what works well, and why it works. These success stories may include reference to macro, meso or micro levels of healthcare systems, various sectors (eg aged care, acute care or primary care), or specific programs or projects. Health System Improvement Across the Globe is unprecedented in terms of both reach and positive emphasis, and as such will be instrumental in changing ways of thinking about and guiding health-sector improvement.

Integrating Human Factors Methods and Systems Thinking for Transport Analysis and Design
Governments and road safety agencies around the world have either introduced or are considering 'safe system' strategies, a long overdue acknowledgement that different elements of the road system contribute to road safety outcomes. Human factors approaches have a leading role here in both conceptualising the road system as a complex sociotechnical system and in providing practical approaches to support true systems-based countermeasures. This book illustrates the potential for integrating contemporary systems-based human factors methods with modern day driving-assessment methods, such as vehicle instrumentation and driving simulation, to understand and enhance performance in modern day road-transport systems. The book outlines why a fundamental paradigm shift is needed in the way these systems are designed and operated, and illustrates how a wide range of accepted human-factors approaches can be applied successfully to road transport to revolutionise the countermeasure design process. The practical illustrations of these human factors methods are applied to a long-standing road and rail safety issue: rail level crossings, where the road and rail systems intersect. The final chapter of the book highlights the utility of the human factors approach to reducing road trauma and discusses future applications of the approach.

A History of Occupational Health and Safety: From 1905 to the Present
The United States has a long and unfortunate history of exposing employees, the public, and the environment to dangerous work. But in April 2009, the spotlight was on Las Vegas when the Pulitzer committee awarded its public service prize to the Las Vegas Sun for its coverage of the high fatalities on Las Vegas Strip construction sites. The newspaper attributed failures in safety policy to the recent exponential growth in the Las Vegas market. In fact, since Las Vegas' founding in 1905, rapid development has always strained occupational health and safety standards. A History of Occupational Health and Safety examines the work, hazards, and health and safety programs from the early building of the railroad through the construction of the Hoover Dam, chemical manufacturing during World War II, nuclear testing, and dense megaresort construction on the Las Vegas Strip. In doing so, this comprehensive chronicle reveals the long and unfortunate history of exposing workers, residents, tourists, and the environment to dangerous work-all while exposing the present and future to crises in the region. Complex interactions and beliefs among the actors involved are emphasized, as well as how the medical community interpreted and responded to the risks posed. Few places in the United States contain this mixture of industrial and postindustrial sites, the Las Vegas area offers unique opportunities to evaluate American occupational health during the twentieth century, and reminds us all about the relevancy of protecting our workers.

Safety-II in Practice: Developing the Resilience Potentials
Safety-I is defined as the freedom from unacceptable harm. The purpose of traditional safety management is therefore to find ways to ensure this 'freedom'. But as socio-technical systems steadily have become larger and less tractable, this has become harder to do. Resilience engineering pointed out from the very beginning that resilient performance - an organisation's ability to function as required under expected and unexpected conditions alike - required more than the prevention of incidents and accidents. This developed into a new interpretation of safety (Safety-II) and consequently a new form of safety management. Safety-II changes safety management from protective safety and a focus on how things can go wrong, to productive safety and a focus on how things can and do go well. For Safety-II, the aim is not just the elimination of hazards and the prevention of failures and malfunctions but also how best to develop an organisation's potentials for resilient performance - the way it responds, monitors, learns, and anticipates. That requires models and methods that go beyond the Safety-I toolbox. This book introduces a comprehensive approach for the management of Safety-II, called the Resilience Assessment Grid (RAG). It explains the principles of the RAG and how it can be used to develop the resilience potentials. The RAG provides four sets of diagnostic and formative questions that can be tailored to any organisation. The questions are based on the principles of resilience engineering and backed by practical experience from several domains. Safety-II in Practice is for both the safety professional and academic reader. For the professional, it presents a workable method (RAG) for the management of Safety-II, with a proven track record. For academic and student readers, the book is a concise and practical presentation of resilience engineering.

Cognitive Work Analysis: Applications, Extensions and Future Directions
Over the past decade, Cognitive Work Analysis (CWA) has been one of the popular human factors approaches for complex systems evaluation and design applications. This is reflected by a diverse range of applications across safety critical domains. The book brings together a series of CWA applications and discussions from world-leading human factors researchers and practitioners. It begins with an overview of the CWA framework, including its theoretical underpinnings, the methodological approaches involved (including practical guidance on each phase), and previous applications of the framework. The core of the book is a series of CWA applications, undertaken in a wide range of safety critical domains for a range of purposes. These serve to demonstrate the contribution that CWA can make to real-world projects and provide readers with inspiration for how such analyses can be practically carried out. Following this, a series of applications in which new approaches or adaptations have been added to the framework are presented. These show how practical applications feedback into the theories/approaches underpinning CWA. The closing chapter then speculates on future applications of the framework and on a series of new research directions required in order to enhance its utility. In emphasising the practical realities of performing CWA, and the real-world impacts it can provide, the book tackles several common misconceptions in a constructive and persuasive way. It provides a welcome demonstration of how CWA can be a powerful ally in tackling complexity-related problems that afflict systems in all areas.

A Definitive Guide to Behavioural Safety: The Definitive Guide
This book makes the case that far too much work undertaken under the banner of 'behavioural safety' is overly person-focused. 'If you can walk on hot coals, you can do anything - so be safe' needs to be dismissed out of hand, but also more advanced techniques based on coaching and empowerment fail to reflect the fact that, as 'Just Culture' models show, the great majority of causes of unsafe behaviour are environmental. Our methodologies mustn't focus on the person with an open mind that there may be an underlying root cause; they must start from the statistically proven assumption that there is an underlying cause. This shift in mindset has a profound impact on the type of methodologies we must lead with, how they are used, how they are perceived, and last but certainly not least, their efficacy. A Definitive Guide to Behavioural Safety is a one-stop guide to all of the core theories and principles that underpin behaviour-based safety. All front-line behaviours that lead to incidents and injury are covered by the term behavioural safety, and getting to grips with the behaviours that might lead people to engage in unsafe or risky behaviour is crucial to prevention. In this book, internationally acclaimed behavioural safety expert Tim Marsh leads the reader through the three main strands: * The awareness approach. * The walk-and-talk approach. * The Six Sigma safety or the Deming-inspired 'full' approach. Going through the very latest innovations in the field, the book covers the systemic approach to safety observation, measurement, intervention and analysis, but also incorporates emotional intelligence training aimed at enhancing supervisor-worker trust and communication more generally. A Definite Guide to Behavioural Safety is a perfect guide for any professional, whether you're aiming to set up an ambitious and wide-ranging behavioural safety programme from scratch or you're looking to refresh or extend an existing approach.

It is a book designed to answer the questions often posed as to why should we do it this way. It is the Why book and the intent is to provide a blueprint and a helpmate for the philosophical basis for occupational safety and health and the justification as an integral component of doing business.

Diagnosis: Interpreting the Shadows
Despite diagnosis being the key feature of a physician's clinical performance, this is the first book that deals specifically with the topic. In recent years, however, considerable interest has been shown in this area and significant developments have occurred in two main areas: a) an awareness and increasing understanding of the critical role of clinical decision making in the process of diagnosis, and of the multiple factors that impact it, and b) a similar appreciation of the role of the healthcare system in supporting clinicians in their efforts to make accurate diagnoses. Although medicine has seen major gains in knowledge and technology over the last few decades, there is a consensus that the diagnostic failure rate remains in the order of 10-15%. This book provides an overview of the major issues in this area, in particular focusing on where the diagnostic process fails, and where improvements might be made.

Creating the Productive Workplace: Places to Work Creatively
The built environment affects our physical, mental and social wellbeing. Here renowned professionals from practice and academia explore the evidence from basic research as well as case studies to test this belief. They show that many elements in the built environment contribute to establishing a milieu which helps people to be healthier and have the energy to concentrate whilst being free to be creative. The health and wellbeing agenda pervades society in many different ways but we spend much of our lives in buildings so they have an important role to play within this total picture. This demands us to embrace change and think beyond the conventional wisdom whilst retaining our respect for it. This book shows how we need to balance the needs of people, the ever increasing enabling technologies but also to take advantage of the healing powers of Nature and let them be part of environmental design.
