When a young child has a severe speech sound disorder-especially one severe enough to affect the child's intelligibility-it's critical to determine the degree to which motor speech impairment contributes to the disorder. Now there's a psychometrically well-supported tool that helps speech-language pathologists do just that. Dynamic Evaluation of Motor Speech Skill (DEMSS) is a criterion-referenced assessment designed to help with differential diagnosis of speech sound disorders in children with severely impaired speech production. Developed by one of the leading experts on childhood apraxia of speech (CAS) and an expert on test development, this tool is an efficient way to assess children who have significant speech impairment, especially reduced phonemic and/or phonetic inventories, vowel or prosodic errors, poor speech intelligibility, and/or little to no verbal communication. USE DEMSS to: - Diagnose severe speech sound disorders in children 3 and older - Facilitate, confirm, or rule out a diagnosis of CAS - Estimate the severity of a child's disorder and what their prognosis might be - Inform the development of treatment goals - Make decisions about the most effective methods of cueing during treatment How It Works: DEMSS is a dynamic assessment administered by SLPs in a clinical setting, usually in about 30 minutes or less. The child is tested on production of 60 utterances, divided into eight grouped sets according to syllable structure. For each item on the DEMSS, the SLP provides a verbal model, asks the child for a direct imitation, and follows up with more cues and supports if the child's first attempt is incorrect. Scoring takes into account overall accuracy in producing the word, vowel accuracy, consistency of production, and accuracy of prosodic features of the word. After administering DEMSS and determining results, SLPs can schedule a follow-up meeting with the child's parents to share results and make recommendations. Healthcare-Associated Infections in Children: A Guide to Prevention and Management
With advances in technology and medical science, children with previously untreatable and often fatal conditions, such as congenital heart disease, extreme prematurity and pediatric malignancy, are living longer. While this is a tremendous achievement, pediatric providers are now more commonly facing challenges in these medical complex children both as a consequence of their underlying disease and the delivery of medical care. The term healthcare-associated infections (HAIs) encompass both infections that occur in the hospital and those that occur as a consequence of healthcare exposure and medical complexity in the outpatient setting. HAIs are associated with substantial morbidity and mortality for the individual patient as well as seriously taxing the healthcare system as a whole. In studies from the early 2000s, over 11% of all children in pediatric intensive care units develop HAIs and this figure increases substantially if neonatal intensive care units are considered. While progress has been made in decreasing the rates of HAI in the hospital, these infections remain a major burden on the medical system. In a study published in 2013, the annual estimated costs of the five most common HAIs in the United States totaled $9.8 billion. An estimated 648,000 patients developed HAIs in hospitals within the US in 2011 and children with healthcare-associated bloodstream infection have a greater than three-fold increased risk of death.While a number of texts discuss HAIs in the broader context of infectious diseases or pediatric infectious diseases (such as Mandell's Principles and Practice of Infectious Diseases or Long and Pickering's Principles and Practice of Pediatric Infectious Diseases) no single text specifically focuses on the epidemiology, diagnosis and management of HAI in children. Many infectious diseases texts are organized based on the microbiology of infection and from this starting point then discussing the clinical syndromes associated with the organism of interest. For instance, a chapter on Staphylococcus aureus may contain a brief discussion of the role of S. aureus in surgical site infections in the wider context of all staphylococcal disease. For clinicians caring for children at the bedside, however, the clinical syndrome is typically appreciated and intervention necessary prior to organism identification. We propose a text that details both the general principles involved in HAIs and infection prevention but also provides a problem oriented approach. Such a text would be of interest to intensivists, neonatologists, hospitalists, oncologists, infection preventionists and infectious diseases specialists. The proposed text will be divided into three principle sections: 1) Basic Principles of Infection Control and Prevention, 2) Major Infectious Syndromes and 3) Infections in Vulnerable Hosts. Chapters in the Major Infectious Syndromes section will include discussion of the epidemiology, microbiology, clinical features, diagnosis, medical management (or surgical management as appropriate) and prevention of the disease entity of interest. Chapters will seek to be evidenced based as much as possible drawing from the published medical literature as well as from clinical practice guidelines (such as those from the Infectious Diseases Society of America) when applicable. We intend to include tables, figures and algorithms as appropriate to assist clinicians in the evaluation and management of these often complex patients. Finally, we intend to invite authors to participate in this project from across a number of medical specialties including infectious diseases, infection control, critical care, oncology and surgery to provide a multidisciplinary understanding of disease. It is our intent to have many chapters be co-written by individuals in different subspecialties; for instance, a chapter on ventilator-associated pneumonia may be co-written by both infectious disease and critical care medicine specialists. Such a unique text has the potential to provide important guidance for clinicians caring for these often fragile children.

Pediatric dysphagia is a clinical problem that crosses disciplines. Children may be seen by numerous medical specialties including pediatric otolaryngology, gastroenterology, pulmonology, speech pathology, occupational therapy, and lactation consultants. The myriad approaches to the diagnosis and management of dysphagia is confusing for both clinicians and families; resulting in recurrent trips to medical professionals. Feeding is integral to socialization and to bonding between infants and parents. Disruptions in feeding development can be extremely taxing emotionally and economically for families. Children with dysphagia are some of the most challenging patients even for clinicians who specialize in their care. This text provides the reader with a comprehensive understanding of swallowing and presents a practical, evidence-based approach to the diagnosis and management of swallowing difficulties in children. It also highlights particular clinical challenges and controversies in the management of pediatric dysphagia. It is unique in that it incorporates the perspectives of multiple types of clinicians that care for these patients including otolaryngologists, gastroenterologists, pulmonologists, speech pathologists, occupational therapists and lactation consultants. In doing so, this text will encourage cross-specialty pollination of ideas and knowledge as well as stimulate further research in the field. Part 1 of the text begins with an overview of the anatomy and physiology of swallowing with a focus on normal development as we currently understand it. It also discusses new information regarding reflexive interactions between the larynx and esophagus that potentially influence swallowing. It then moves on to a discussion of the advantages and limitations of currently available diagnostic modalities and highlights current controversies regarding frame rate, radiation exposure, breastfeeding infants, and grading of studies. Additionally, it reviews the current literature regarding medical and behavioral-based therapy options, including thickening options, oromotor therapy, and controversies concerning strict NPO. Part 2 addresses specific diagnoses which can cause or be associated with dysphagia such as prematurity, velopharyngeal insufficiency, ankyloglossia, laryngeal clefts, laryngomalacia, vocal fold paralysis, and cricopharyngeal dysfunction. The text goes on to explore the pathophysiology and treatment options for each. Anatomic, inflammatory, and neuromuscular esophageal causes of dysphagia are also evaluated. In addition, it delves into the impact of craniofacial anomalies, sialorrhea and psychological factors on swallowing. Finally, it discusses how a multidisciplinary aerodigestive team can help streamline multidisciplinary care for individual patients.
