This textbook offers a comprehensive analysis of medical decision making under uncertainty by combining Test Information Theory with Expected Utility Theory. The book shows how the parameters of Bayes' theorem can be combined with a value function of health states to arrive at informed test and treatment decisions. The authors distinguish between risk-neutral, risk-averse and prudent decision makers and demonstrate the effects of risk preferences on physicians' decisions. They analyze individual tests, multiple tests and endogenous tests where the test outcome is chosen by the decision maker. Moreover, the topic is examined in the context of health economics by introducing a trade-off between enjoying health and consuming other goods, so that the extent of treatment and thus the potential improvement in the patient's health becomes endogenous. Finally, non-expected utility models of choice under risk and uncertainty (i.e. ambiguity) are presented. While these models can explain observed test and treatment decisions, they are not suitable for normative analyses aimed at providing guidance on medical decision making.

This book takes an approach to evidence synthesis that is specifically intended for decision making when there are two or more treatment alternatives being evaluated, and assumes that the purpose of every synthesis is to answer the question for this pre-identified population of patients, which treatment is 'best'? Crucially, this epidemic marked the first time the virus was able to spread beyond rural areas to major cities, overturning conventional assumptions about its epidemiology. With backgrounds ranging from development to disease control, the contributors to this volume - some of them based in countries affected by the Ebola epidemic - consider the underlying factors that shaped this unprecedented outbreak. While championing the heroic efforts of local communities and aid workers in halting the spread of the disease, the contributors also reveal deep structural problems in both the countries and humanitarian agencies involved, which hampered the efforts to contain the epidemic. Alarmingly, they show that little has been learned from these events, with health provision remaining underfunded and poorly equipped to deal with future outbreaks. Such issues, they argue, reflect the wider challenges we face in tackling epidemic disease in an increasingly interconnected world.

Drawing on fifty years of experience caring for children and adults, Dr. Moskowitz examines vaccines and our current policy regarding them. Weaving together a tapestry of observed facts, clinical and basic science research, news reports from the media, and actual cases from his own practice, he offers a systematic review of the subject as a whole. He provides scientific evidence for his clinical impression that the vaccination process, by its very nature, imposes substantial risks of disease, injury, and death that have been persistently denied and covered up by manufacturers, the CDC, and the coterie of doctors who speak for it. With the aim of acknowledging these risks, taking them seriously, understanding them more holistically, and ultimately assessing them on a deeper level, he proposes a nationwide debate based on objective scientific research, including what we already know and what still needs to be investigated in the future. He argues that with no serious public health emergency to justify them, requiring vaccines of everyone deprives us all of genuinely informed consent, and prevents parents from making health-care decisions for our children, basic human rights that we still profess to hold dear. For the present, given the legitimate controversy surrounding the mandates, he proposes that most vaccines simply be made optional and that further research into their risks and benefits be conducted by an independent agency in the public interest, untainted by industry funding, CDC sponsorship, and the quasi-religious sanctimony that is widely invoked on their behalf.
