Osteoporosis: Pathophysiology and Clinical Management
Completely revised and updated, and utilizing the most current evidence and practice guidelines for the treatment of osteoporosis, this comprehensive third edition discusses the basic aspects of bone metabolism, the pathophysiology of osteoporosis, current diagnostic techniques and medical treatment strategies. Osteoporosis is a common disorder that is prevalent in over 20 million Americans over the age of 60. One and a half million osteoporotic fractures occur in the United States every year, including 300,000 hip fractures. Mortality rates after hip fracture approach 25%, with another 50% of patients experiencing a major decrease in their prior level of independence and quality of life. Despite the wide prevalence and severe consequences of osteoporosis, it remains a disorder that is severely under-diagnosed and treated. In this context, specialists and primary care physicians alike are having increasing difficulty keeping up with the rapid changes to the field and incorporating these advances to clinical care. In the years since the last edition of this book was published, the osteoporosis field has changed drastically. In addition to revising and updating existing chapters and removing a few that are no longer as relevant, new chapters discuss an advanced understanding of the cellular and molecular mechanisms underlying the disorder, the introduction of new diagnostic imaging techniques, a more nuanced appreciation of the risks and benefits of osteoporosis therapies, and the introduction of two new classes of osteoporosis medications. Following the format of the second edition, and including helpful key points at the opening of each chapter, this text will present a comprehensive overview of both the basic and clinical concepts relating to each topic, when appropriate. Chapter authors were chosen based on their high level of expertise and leadership in the field. Taken together, this text should thus be of great interest to physicians of multiple specialties, allied health professionals, as well as basic and clinical researchers.

Behavioral Diabetes: Social Ecological Perspectives for Pediatric and Adult Populations
This book presents an up-to-date review of behavioral factors in diabetes management across the lifespan: an update on medical management, epidemiology, and prognosis, and utilize an ecological framework to address various aspects of diabetes management for children and adults on the individual, social, community and medical system, and policy levels. The individual level examines biobehavioral and neuroendocrine factors for their role in the etiology of diabetes, as well as various demographic factors involved in health disparities, and specific psychological issues including distress and quality of life, depression and anxiety, eating disorders, and intervention approaches. Zooming out, the social level addresses the role of social support and family influences as well as group and family interventions to promote more effective diabetes management. The community level addresses medical system factors including the patient-physician relationship and transition programs, as well as community and school-based prevention programs. Finally, chapters also address how the policy level impacts diabetes management considering the role of health care, insurance, and school and workplace policy. Topics featured in this book include: Neuroendocrine and biobehavioral influences on diabetes Eating disorders in individuals with diabetes Family influences and family therapies for children and adults with diabetes Depression and anxiety in children and adults with diabetes Behavioral Diabetes is a must-have resource for researchers, graduate students and fellows, as well as clinicians, therapists, and other practitioners involved in diabetes management across the lifespan. A secondary audience comprises individuals working in the community and policy levels, including but not limited to health care and medical systems administrators, as well as school and workplace policymakers. This book is a comprehensive overview of the extremely important topic of behavioral diabetes. The issues encompassed in this book have evolved greatly over the last few decades and the editors have done a spectacular job in having the key experts on each of the many topics review the literature while at the same time keeping it practical for both clinicians and researchers. --Irl B. The Power of Fastercise: Using the New Science of Signaling Exercise to Get Surprisingly Fit in Just a Few Minutes a Day
A revolutionary program of short burst, high-intensity exercise that uses your body's signals to curb hunger as it burns fat and builds muscle Over the last 26 years, thyroid pioneer Denis Wilson, MD, has trained thousands of physicians on the crucial relationships between the thyroid system, metabolism, and body temperature. He's heard patients recount their inability to get fit using conventional approaches, and he's understood their frustration. Based on the latest medical research, Dr. Wilson has created fastercise, a revolutionary practice that uses brief, strategically timed bursts of exercise to cancel hunger pangs, allowing people to more easily stick to a healthy eating plan and shift their bodies toward becoming leaner, faster, smarter, stronger, and healthier. Fastercise holds the promise of vindicating and liberating many of those who have struggled to improve their fitness, enabling them to transform their lives and reach their full potential. By combining simple analogies and clear explanations of the physiology of the body's energy pathways and response to food and exercise, Dr. Wilson reveals how conventional approaches to dieting and weight management can actually fight against the body's priorities and lead to frustration and poor results. Fastercise is a time-efficient, convenient, and natural approach powerfully signals the body to burn fat and build muscle synergistically, leading to surprisingly beneficial and quick results. The Power of Fastercise explains how fastercise can help you: - Burn fat without going hungry - Build your mitochondria to burn more fat and provide greater energy - Stimulate muscle growth in just a few minutes a day - Shift your body composition to less fat and more muscle - Boost your body temperature and metabolic rate - Look and feel younger - Increase mental focus, learning, and productivity - Decrease insulin resistance - Decrease inflammation and improve immune function - Improve respiratory fitness and athletic performance - Get great results with any healthy diet, including low-carb and high-carb In this groundbreaking book, Dr. Wilson lays out simple, practical strategies for combining fastercise with smart eating choices. Fastercise can provide excellent results for a wide range of people: seasoned athletes, fitness enthusiasts, and even those who dislike exercising or have physical limitations. Whatever your fitness goals are, fastercise can help you achieve them.

The Diabetes Textbook: Clinical Principles, Patient Management and Public Health Issues
Diabetes has become a worldwide health problem, the global estimated prevalence approaches ten percent and the burden of this disease in terms of morbidity and mortality is unprecedented. The advances acquired through the knowledge of the mechanisms of the disease and the variety of therapeutic approaches contrast with the inability of private and public health systems in underdeveloped and even developed countries to achieve the goals of treatment. This paradox has been described in many sources: the surge of scientific advances contrast with an unprecedented amount of human suffering. Thus, a patient centered and an evidence based approach with the capacity to produce measurable clinical and economic outcomes is required. The purpose of this textbook is multiple: to offer a comprehensive resource covering all aspects of outpatient management; to address diabetes as a health problem from an epidemiological, economic and clinical perspective; to discuss the role of social determinants of health on the worldwide increase in diabetes; to highlight the challenges and obstacles in providing adequate care; and to outline a multidisciplinary approach to management in which medical visits retain their importance as part of a team comprising the patient, his or her family and a multidisciplinary group of health professionals who are able to move beyond the traditional approach of diabetes as a disease and greatly improve outcomes.

Diabetic Foot: Prevention and Treatment
This book first proposes a noninvasive screening method for diabetes based on the thermoregulation of the peroneal vessel. Since diabetes affects the proneal vessel of the patients significantly, the thermoregulatory behaviour of peroneal vessel is studied for induced hot and cold stress in this work. Next, the authors highlight recent findings in the area of human mesenchymal stem cells sources, their differentiation ability, immunogenicity, adaptation to the microenvironment, as well as use in human clinical trials. The authors also propose that, given the increasing prevalence of antibiotic resistant pathogens and the failure of antibiotic-exclusive therapeutics in the treatment of diabetic foot infections, combinations of antimicrobial peptides and antibiotics may be a potential treatment alternative. Advanced diabetic foot ulcer therapies are explored based on current research. Recent studies show that diabetic patients have a 25% risk of developing diabetic foot ulcers in their lifetime. Lastly, a study was carried out to investigate the effects of social support on the reduction of wound size after four weeks of treatment with standard care in patients with Grade B, Stage I diabetic foot ulcer.

Recurrent Implantation Failure
Implantation is a complex phenomenon, still not thoroughly understood, involving the embryo and the endometrium. Successful implantation is considered to stem from an efficient combination of various embryo and/or maternal factors. Repeated failure of any of these factors or of their combination might decrease the chance of implantation and eventually lead to recurrent implantation failure. Despite technological advances, only about a quarter of In Vitro Fertilization procedures will result in live birth, leading many couples to experience multiple failures. This text from international experts explores the various factors at play and offers a helpful summary of the state of knowledge to help guide clinicians in their management of this complex and important problem. Contents: What is Recurrent Implantation Failure? * The psychosocial aspects of recurrent implantation failure * The role of life-style factors in RIF * Endocrine causes of recurrent implantation failure * Congenital uterine anomalies and recurrent implantation failure * Immunological causes of recurrent implantation failure * Acquired uterine conditions, reproductive surgery, and RIF? * Thrombophilia: diagnosis and management in women with recurrent implantation failure * Andrological causes of RIF * Diagnostic evaluation of RIF? (Modalities and algorithm) * Optimizing embryo culture for RIF? * Optimizing endometrial receptivity for patients with recurrent implantation failure: The role of progesterone on the day of triggering final oocyte maturation * Is there a role of follicular phase LH and E2 on the day of triggering final oocyte maturation for embryo implantation? * Manipulating the endometrium: Can RIF be managed by inducing endometrial inflammation or by the use of novel research therapies * The embryo in RIF: Genetic selection and strategies for improving its implantation potential * Is oocyte donation efficient in patients with RIF? * Sperm donation and RIF? * Optimizing embryo transfer technique for RIF management * Proposed management of patients with recurrent implantation failure and directions for future research * When should patients abandon treatment?

Bariatric Robotic Surgery: A Comprehensive Guide
The present book intends to provide a comprehensive guide to the field of robotic bariatric surgery. It covers all the stages and procedures needed to fulfill credentialing for performing robotic surgery. Also, robotic surgery is presented as an institutional program, and we describe how to establish a robotic program in a hospital environment. The currently accepted and most common procedures - sleeve gastrectomy, gastric bypass and duodenal switch - are described in detail, with a step-by-step description of the techniques, followed by a wealth of photos and videos for each case. Special attention is given to the employment of robotic bariatric surgery in exceptional conditions, such as in super-obese patients, reoperations and revisional procedures. Critical issues, for the success of the robotic surgical interventions, such as anesthesia, are also addressed. Finally, the outcomes of robotic bariatric surgery are described, including long-term weight loss, improvement and resolution of comorbidities and improvement in quality of life. Bariatric Robotic Surgery is the first book specially devoted to this modality of surgical intervention. It is a fundamental tool for surgeons, residents and fellows who want to start a robotic bariatric surgery program. The book also helps experienced robotic surgeons to keep up to date with the various available robotic surgical techniques.

Rethinking Diabetes: Entanglements with Trauma, Poverty, and HIV
In Rethinking Diabetes, Emily Mendenhall investigates how global and local factors transform how diabetes is perceived, experienced, and embodied from place to place. Mendenhall argues that the link between sugar and diabetes overshadows the ways in which underlying biological processes linking hunger, oppression, trauma, unbridled stress, and chronic mental distress produce diabetes. The life history narratives in the book show how deeply embedded these factors are in the ways diabetes is experienced and (re)produced among poor communities around the world. Rethinking Diabetes focuses on the stories of women living with diabetes near or below the poverty line in urban settings in the United States, India, South Africa, and Kenya. Mendenhall shows how women's experiences of living with diabetes cannot be dissociated from their social responsibilities of caregiving, demanding family roles, expectations, and gendered experiences of violence that often displace their ability to care for themselves first. These case studies reveal the ways in which a global story of diabetes overlooks the unique social, political, and cultural factors that produce syndemic diabetes differently across contexts. From the case studies, Rethinking Diabetes clearly provides some important parallels for scholars to consider: significant social and economic inequalities, health systems that are a mix of public and private (with substandard provisions for low-income patients), and rising diabetes incidence and prevalence. At the same time, Mendenhall asks us to unpack how social, cultural, and epidemiological factors shape people's experiences and why we need to take these differences seriously when we think about what drives diabetes and how it affects the lives of the poor.
