Post-traumatic Stress Disorder
Despite the growing interest in the role of psychological trauma in the genesis of psychiatric disorders, few volumes have addressed these issues from a multidisciplinary and international perspective. Given the complexity of reslience and posttraumatic disorder, and given ongoing trauma and violence in many parts of the world, it is crucial to apply such perspectives to review existing knowledge in the field and provide directions for future research. This book has a broad scope. A key focus is PTSD, because of its clinical and health importance, its obvious link with trauma, and its interest for many clinicians and researchers. However, the book also examines resilience and a range of mental health consequences of trauma, because it has become increasingly clear that not all individuals react to trauma in the same way. It is important for mental health professionals to be aware of the broad range of potential responses to trauma, as well as of relevant evidence-based treatments. The book includes chapters that address a wide range of topics on trauma-related disorders, including nosology and classification, epidemiology, neurobiology, pharmacotherapy, and psychotherapy. Each chapter comprises a critical review of the existing literature, aimed at being useful for the practitioner. This is followed by selected commentaries from other authorities on the topic, representing diverse geographical locations and points of view, who refine some of the perspectives offered in the review, provide alternative views, or suggest important areas of future work.

Community Mental Health: Putting Policy Into Practice Globally
There are wide inconsistencies between, and even within, countries in how community-orientated care is defined and interpreted. The analysis presented in this book take as a starting point an evidence-based balanced care model in which services are provided in community settings close to the populations served, with hospital stays being reduced as far as possible, usually located in acute wards in general hospitals. The surprising conclusion from the research is that the same problems arise in all countries, regardless of resource status, and thus the recommendations of this book apply to mental health provision everywhere. This book reviews the implementation of community-orientated care using the balanced care model. It summarizes the steps, obstacles and mistakes that have been encountered in the implementation of community mental health care worldwide and presents guidelines on how to avoid them. It proposes realistic and achievable recommendations for the development and implementation of community-orientated mental health care over the next ten years. These guidelines will be of practical use to psychiatrists and other mental health and public health practitioners at all levels worldwide, including policy makers, commissioners, funders, non-governmental organisations, service users and carers. A core message of the book is that the mental health sector will more powerfully advocate for better services in future through strong and unified alliances, especially with powerful representation from consumer/service user and carer groups. Community-orientated care draws on a wide range of practitioners, providers, care and support systems (both professional and non-professional), though particular components may play a larger or lesser role in different settings depending on the local context and the available resources, especially trained staff. Research by a WPA task force has demonstrated that most of the challenges are common and global, but with local variations. The book is therefore relevant to psychiatrists and mental health workers in developed countries who are trying to deliver better health care on reduced budgets and for those in the developing economies who are in the position to modernise their mental health care. It provides clear, concise guidance on policy and practice decisions, learning from what has and has not worked in regions in the world. The book contains many tables documenting the evidence, supported by an essential reference list, and a Key Points summary for each chapter. Highly Commended in the Psychiatry section of the 2012 BMA Book Awards.

Eating Disorders and the Brain
Why is the brain important in eating disorders? This ground-breaking new book describes how increasingly sophisticated neuroscientific approaches are revealing much about the role of the brain in eating disorders. Even more importantly, it discusses how underlying brain abnormalities and dysfunction may contribute to the development and help in the treatment of these serious disorders. * Neuropsychological studies show impairments in specific cognitive functions, especially executive and visuo-spatial skills. * Neuroimaging studies show structural and functional abnormalities, including cortical atrophy and neural circuit abnormalities, the latter appearing to be playing a major part in the development of anorexia nervosa. * Neurochemistry studies show dysregulation within neurotransmitter systems, with effects upon the modulation of feeding, mood, anxiety, neuroendocrine control, metabolic rate, sympathetic tone and temperature. The first chapter, by an eating disorders clinician, explains the importance of a neuroscience perspective for clinicians. This is followed by an overview of the common eating disorders, then chapters on what we know of them from studies of neuroimaging, neuropsychology and neurochemistry. The mysterious phenomenon of body image disturbance is then described and explained from a neuroscience perspective. The next two chapters focus on neuroscience models of eating disorders, the first offering an overview and the second a new and comprehensive explanatory model of anorexia nervosa. The following two chapters offer a clinical perspective, with attention on the implications of a neuroscience perspective for patients and their families, the second providing details of clinical applications of neuroscience understanding. The final chapter looks to the future. This book succinctly reviews current knowledge about all these aspects of eating disorder neuroscience and explores the implications for treatment. It will be of great interest to all clinicians (psychiatrists, psychologists, nurses, dieticians, paediatricians, physicians, physiotherapists) working in eating disorders, as well as to neuroscience researchers.

The Molecular Biology and Pathology of Elastic Tissues
Elastin is a molecule of the extracellular matrix, which appeared during phylogenesis only with the vertebrates. Organs and tissues rich in elastin (e.g. in the circulatory and respiratory systems) age the most rapidly. Elastic fibres are complex structures and their biosynthesis presents problems for the elastin-producing cell. Physiological turnover of insoluble elastin is very slow, so little remodelling of elastic fibres occurs during adult life. However, increased elastin destruction occurs in certain pathological conditions, due either to the release of elastases from inflammatory cells or bacteria, or to a genetic deficiency in an elastase inhibitor. Membrane-associated elastin-binding proteins have been discovered recently. These have a high affinity for tropoelastin (the soluble elastin precursor) and for fragments of insoluble elastin. Also, they mediate a variety of cellular reactions, including chemotaxis, release of lytic enzymes or oxygen radicals, and cell adhesion. They may also play a role in the movement of inflammatory cells and metastic tumour cells through elastin-rich tissues. With the cloning of the genes for tropoelastin and several microfibrillar proteins, as well as the discovery of elastin receptors, research on this topic is proceeding rapidly. This book includes contributions from a diverse group of scientists and clinicians working on elastin biology. Specific topics include the structure and function of elastin, its molecular biology and regulation, and its role in ageing and pathology.
